Appointments & Scheduling:
Patients are required to schedule appointments in advance. Walk-ins may be accommodated based on provider availability.
Cancellations must be made at least 24 hours prior to the scheduled appointment. Late cancellations or missed appointments may result in a fee of $50. -
Payments:
Payment is due at the time of service unless prior arrangements are made. We accept cash, debit/credit cards, and approved insurance plans. Patients are responsible for any balances not covered by insurance. -
Insurance:
While Prime Family Medicine and Wellness Clinic LLC will submit claims to contracted insurance providers, patients are responsible for understanding their own coverage and for all charges not covered by insurance. -
Privacy & Confidentiality:
All patient information is handled in compliance with HIPAA regulations. Medical records will not be released without written consent, except as required by law. -
Treatment Refusal:
Prime Family Medicine and Wellness Clinic LLC reserves the right to refuse treatment if it is deemed medically inappropriate, unsafe, or outside the clinic’s scope of practice. Patients also have the right to refuse treatment at any time.

Prime Family Medicine and Wellness Clinic LLC is not an emergency facility. In case of a medical emergency, patients should call 911 or go to the nearest emergency department.
