PRE-OP CLEARANCE VISIT
A Pre-Operative Clearance Visit (also called Pre-Op Clearance or Pre-Surgical Evaluation) is a medical appointment performed before a planned surgery or procedure. The goal is to ensure that the patient is healthy enough to safely undergo anesthesia and surgery.
During this visit, a healthcare provider will:

SPORTS PHYSICAL
Sports Physicals, also known as pre-participation physical exams, are designed to make sure athletes are healthy and ready to safely take part in physical activities. These exams are often required by schools, sports teams, and athletic programs before participation.

A typical sports physical includes a review of medical history, vital signs, vision check, heart and lung evaluation, joint and muscle assessment, and discussion of any past injuries or ongoing conditions. SCHOOL FORMS (2131, 3300)
School Health Forms (Form 3231 & Form 3300)
In Georgia, children are required to have certain health forms completed before starting school or daycare. At Prime Family Medicine & Wellness Center, we make this process simple and convenient for families.

Form 3231 – Certificate of Immunization
This form documents that your child has received all state-required vaccinations. Our providers review your child’s immunization record and update vaccines if needed to ensure compliance with Georgia school entry requirements. -
Form 3300 – Certificate of Vision, Hearing, Dental, and Nutrition Screening
This form confirms that your child has been screened for vision, hearing, dental health, and nutrition. These screenings help identify concerns early so your child can succeed in school with the best possible health support.
Both forms are typically required for school enrollment, pre-K, kindergarten, and daycare programs. We provide thorough evaluations, explain results to parents, and complete the official Georgia Department of Public Health forms during the same visit whenever possible.

SMALL SUTURES (stitches) used to close wounds, cuts, or surgical incisions. They are made from thin suture material (such as nylon, polypropylene, silk, or absorbable types like polyglycolic acid) and are placed with a needle to hold the edges of skin or tissue together while healing occurs.
Because they are small, these sutures are typically used for delicate areas (like the face, lips, or small lacerations) where precision and minimal scarring are important. They provide just enough strength to keep tissues approximated without causing unnecessary tension or large stitch marks.
INCISION AND DRAINAGE (often abbreviated as I&D) is a minor surgical procedure in which a healthcare provider makes a small cut (incision) into the skin or tissue to open up and release the collection of pus, fluid, or blood (usually from an abscess, cyst, or infected wound).
The main steps include:

Incision: Using a sterile scalpel or blade to open the abscess or fluid pocket.

Drainage: Allowing the pus or fluid to escape, relieving pressure and pain.

Irrigation/packing (if needed): The cavity may be washed with sterile solution and sometimes lightly packed with gauze to keep it open and prevent reaccumulation.
